COVID-19 VACCINE PASSPORT

The Covid Passport

The Covid-19 passport in England is available via the NHS APP.  This is different to the NHS app used for track and trace.  For more information about how to download the NHS APP click here.  For those people who do not have access to the NHS app or do not wish to use they can call 119 or visit the NHS website to request a paper certificate.  This will take a minimum of 5 days to be delivered. People should wait five working days AFTER they have had their second dose for the system to be updated.

Missing or incorrect Vaccination Information

If you have a query about missing or incorrect vaccination information then you must speak with the vaccination site where you had your vaccination.  Your GP is not able to correct or add information unless the vaccination took place at Chorleywood Health Centre.

Vaccines administered overseas

From 9 December, if you live in England and have had one or more of your COVID-19 vaccinations administered overseas, you can have your vaccination details added to your NHS Record. You can then generate an NHS COVID Pass for domestic use and international travel if you meet certain certification requirements.

Eligible vaccines

You can use this service if you’ve had one or more of the following vaccines:

  • Moderna
  • AstraZeneca
  • Pfizer
  • Janssen

There will remain a small number of countries and territories where this service will not be enabled. However, international COVID-19 certification accepted at the UK border is also accepted at events and venues in England.

Request for Letters

We have had many enquiries from patients to provide letters to show that they have tested positive for Covid 19 and as such are not able to take a PCR test prior to travel or for an event. Chorleywood Health Centre will not be providing letters as this is outside of our contacted core work.

If you require evidence of your positive test we would advise you to either view this on your NHS App / Patient access or request a print out of the positive result from your GP.    You can request this by clicking on the following link and completing the request online.
