Carers

The latest Carewaves magazine is out now and is available to download or read on the website.  The magazine was completed as restrictions from England’s third coronavirus lockdown were easing, so some of the face to face services, such as carers’ groups and mentoring, may have resumed or will do so shortly, rather than being offered over the phone or online. Please contact Carers in Hertfordhshire if you would like support with your caring role or for up to date information about our services. Email contact@carersinherts.org.uk or call 01992 58 69 69.

Register as a Carer

Are you looking after or providing support for a relative, friend, or neighbour? Are you being helped or supported by a relative, friend, or neighbour?

Your GP needs to know so you can be offered the right information, support and access to services. GP surgeries have to compile information for the government about the numbers of carers attached to the surgery so your information will help them to do this.

Benefits of registering as a carer at your GP surgery

We as a practice are dedicated to offering you as much help & support as needed & we work very closely with Carers in Hertfordshire in order to achieve this. Once registered as a carer we can offer you if required:

  • Flexible Appointments
  • Flu vaccination
  • Covid-19 Vaccination
  • Information & Support
  • Referral to Carers in Hertfordshire

Please click here to print the form or complete the online form below. Return any downloaded forms to the surgery so that we can register you as a carer on our clinical system. Alternatively please collect a form from reception or speak to a clinician during your next appointment.  For further information please ask to speak to the Carer Champion at the Health Centre

There is more information about help with day-to-day living because of illness or disability on the Carers in Hertfordshire website or on the NHS website.

Carers survey

We would like to get feedback from carers about how we might improve  the support that Health and Social Care services are providing to carers. Please click here to access the survey. Once completed, please return the survey to the Reception team at Chorleywood Health Centre.  We will not be able to identify you from the information you give, unless you give your name and address at the end of the form.  You should only do this if you want a response from us to an issue you have.
